VCO tuning for 8T49N242

I am planning on using an 8T49N242 Clock generator in a design I am working on, but have a few questions about its limitations and uses.

First, how do I tune the VCO frequency, I can see via the app note about programming the 8T49N24X that its VCO can be set between 3 and 4 GHz, but I do not see where to set that/how to tune it.

Second, how accurately can that VCO frequency be tuned? can it be set to any integer Hz between 3 and 4 GHz? are there a set of quantized frequencies that it can achieve?

Finally, if I wish to order this clock generator with some OTP configuration pre-set, is there a minimum order quantity needed to do this?
